The main office is located inside Midland Memorial Hospital, 3rd floor, 432-681-3100, option 2 to make an apt.
Our satellite office is located at the Coleman Family Clinic, 801 E. Florida, 432-685-0450, option 3 to make an appt.
Both locations have an eligibility office located on site to assist them with applying for financial aid if needed.
