The Healthcare Network of Southwest Florida uses a sliding -fee scale based on proof of income and individual patient's ability to pay.
They can assist you in applying for Medicaid benefits, insurance, and reassign PCPs.
Also accept payment from third-party payers such as Commercial Insurance, Health Maintenance Organizations (HMOs) with authorizations, Medicare & Medicaid.
To assist you in qualifying for health benefit programs, please take the following information (if applicable) to the Primary Care Center of your choice:
Proof of income:
Pay stubs (last four weeks of income)
Last Year's Income Tax Return or Schedule C if self-employed
Workers Compensation (last four weeks of income)
Unemployment Compensation (last four weeks of income)
Disability or SSI Benefits letter
Social Security Income
Documents from local, state, or federal agency
Letter of Support (notarized signature)
Completed Employment Verification
Medicaid made application or rejection letter
Picture identification:
Drivers license
I-94 (If foreign born)
Passport
Determinations are not made until the required documentation has been provided.
